SAVE THE MENISCUS!

HOW TO PRESERVE THE FUTURE OF YOUR KNEE

In 2010, the first edition of the International Meniscus Congress was celebrated in Ghent, Belgium. The objective of this meeting was to unify and update all the international knowledge about the meniscus. In this meeting a clear and forceful message was coined “save your meniscus.”

But, what is the meniscus?

The meniscus is a “pad” that is located inside the joint, between the femur and the tibia. We have two meniscus: internal or medial meniscus and the external or lateral meniscus. Its function is to absorb shock impact between the articular surfaces, increasing the contact surface and allowing a better distribution of loads. Following knee trauma, usually by combining a flexion and rotation movement, a meniscal tear can occur. The appearance of these injuries is usually frequent in sports competition.

Currently, two surgical procedures can be performed for this type of injury:

  • Selective or partial menisectomy, which is based on the removal of only the damaged fragments, or
  • Meniscal suture or repair, attempts to sew the damaged fragments in order to maintain the integrity and structure of the meniscus.

The natural evolution of a knee without a meniscus is to degenerative osteoarthritis. It is therefore important to assess the meniscal injury and try to repair it in order to avoid future osteoarthritis.   But not all meniscal injuries can be repaired. Meniscal suture or repairability depends on several factors; Among them, the type of breakage * (words deleted), its location, associated cartilage lesions etc.

The location of the lesion is important due to the blood supply to the meniscus.  That is to say, for a menisus suture to heal, blood supply to it is necessary. We divide the meniscus into three zones according to the blood supply: red-red zone, red-white zone, and white-white zone.
